Pediatric Ameloblastic Fibro-Odontosarcoma of the Mandible: A Challenge of Diagnosis and Treatment

Report of Case

A 6-year-old girl was referred to the authors' medical center from an eastern European country. Her medical records showed that, 3 years before her admission, an asymptomatic swelling surrounding the left mandible was observed. No further inquiry and treatment were performed at that time. As the facial deformation increased, the parents sought treatment. An incisional biopsy was obtained, with a diagnosis of AFOS. The patient received 3 courses of chemotherapy. No clinical improvement was noted

Discussion

The group of malignant odontogenic tumors consists of carcinoma and sarcoma variants and, very rarely, a carcinosarcoma variant. Malignant ameloblastoma retains a benign morphology but metastasizes. The malignant transformation of the epithelial component is observed in ameloblastic carcinoma, whereas the sarcomatoid transformation of the ectomesenchymal component is observed in AFS, ameloblastic fibrodentinosarcoma, and AFOS.2 In these sarcoma variants, the epithelial component is benign,
